Choosing the right place to bring an aging loved one is a difficult decision.

At Bethel Manor we understand your concerns and want to help you. We care for and about our residents. Bethel Manor is a small, not-for-profit facility on the north side of Evansville, and has served the Tri-State since 1949.

Admitting From Home

  • Pre-Admission Screening (PAS) is required in Indiana to ensure that all alternatives to admission in a nursing facility have been exhausted. Anyone seeking nursing facility placement MUST contact SWIRCA at 812 492 7443 or email at pas16@swirca.org to get this process started.
  • Check out medicare.gov to find out nursing facilities near you. Also, this website provides critical information about nursing homes based on how they staff, how they care for their Residents, results of their health inspections, and any complain inspections that have been conducted in the last year.
  • Once you’ve had a chance to narrow your search down to 2-3 nursing facilities, ask your doctor, friends, family, and anyone else about what they know about these places. Word of mouth is a great way to help you make the right decision.
  • Schedule a tour or just stop by. Touring will allow you to see many different features and characteristics first hand.  Ask yourself- do the Residents look happy and well taken care of?  Do the employees greet you and make eye contact?  Does the facility look clean and smell good.  Also, this is a great opportunity to ask questions ie how long has your Administrator and Director of Nursing worked here?  Key leadership roles such as these can directly impact you or your loved one’s care and services.
  • After you’ve made a decision, contact the Admissions department at that facility. They will conduct an assessment of their own to determine if they can accommodate you or your loved one’s care needs.  Once a decision has been made, the Admissions Coordinator can assist with a smooth transition coming from home.   • Most facilities will require a 30 day payment up front. However, if Medicaid is or will become the main source of payment, please make sure that the facility you have chosen is Medicaid certified.  If you need to apply for Medicaid, ask the Admissions Coordinator for the list of required documents needed for application.  You can also apply for Medicaid online at in.gov/fssa.

Admitting from the Hospital

  • If the hospital doctors and therapists feel that additional services are needed around the clock for you or your loved one, then the case management team at the hospital will assist you with that next step. They will give you a list of facilities to choose from.

  • Once you’ve picked your facility of choice, let the case management team at the hospital know your decision. From there, they will coordinate with the Admissions department.

Benefit coverage will depend on a few factors.  Generally, if a 3 night inpatient hospital stay have occurred, traditional Medicare will pay for nursing home services at 100% for the first 20 days.  If you have specific questions, please do not hesitate to call us.

Transferring from Another Facility

  • Let the social worker at your current facility know that you would like to transfer to another facility. From that point, he/she can provide a list of facilities in the area for you to choose from.

  • The facility you would like to transfer to will conduct an assessment to determine if they can accommodate your needs. Typically, this process should take 1-2 days.  If it is determined that they can meet your needs, the social worker and admissions coordinator will work out the details and the transition can take place fairly quickly.
